Target population is 50-75 year olds who need to get colorectal cancer screening. The project is being done with exact sciences, company who send out cologuard kids for at home screening through the company Elevance healthcare insurance. The members get a call from Elevance team, incentive of gift card if they complete and it closes the HEDIS quality metric. Please refer to first two assignments to build upon information.
